Mediation Appointment Request

Compliance with the statements below is required before submitting your appointment request. Checking the box next to each statement indicates your acknowledgement, understanding and agreement.

We understand that an e-mail confirmation of the scheduled date and time of our mediation will come to each e-mail provided and that will be the only confirmation we will receive from the court.

If you cannot check the boxes above then please do not submit your request for a mediation appointment electronically. You will need to consult with the other party prior to making the appointment unless there is a restraining order prohibiting contact.
