You may print the form then later type in or fill in with ink all of the fields of information that apply to your concern and the issue(s) you have identified or you can electronically fill out the form by tabbing through each field filling in the appropriate information that applies to your concern and issue(s). For date and phone number fields enter the numbers and tab, the formatting will be handled for you. After filling out the form, simply print the document, sign it attesting to the information and mail to the address noted on the form. You are encouraged to attach additional information or documents that contribute information to your concern.
